UTVA Child Find ​Transition Assessments

The individuals with disabilities education act requires us to complete annual transition assessments on all students with active IEP’s  that are in Middle School or High School. This information is used in IEP planning and preparation for post-high school activities such as college or entering the workforce. 
We ask that you complete these surveys as soon as possible, so that if the IEP team decides that you are eligible we are able to use this data to formulate an IEP for your student. Please note that this in no way affects the eligibility process, but is needed for the IEP. This information is kept confidential and will only be used for purposes of IEP planning.
